I - 5 HOSPITAL AND HOME CARE IN ALS PATIENTS IN SMALL COMMUNITIES IN THE U.S.A

ALS is a progressive disease affecting motor function and resulting in eventual immobility and death due to respiratory and bulbar muscle dysfunction. Life may be prolonged in several ways-including alternate ways of obtaining nutrition, respiratory support, and home nursing care or assistance. In rural communities, such as in the state of West Virginia in the USA, patients may be a long distance from a doctor knowledgeable about their disease. Community agencies may also have minimal knowledge or experience with ALS and necessary equipment may not be readily available.
This talk will review the approach to care of ALS patients in a rural setting, including viewpoints of the patient, caregiver, home health agencies, and health care providers. Specific issues relating to the rural environment will be stressed.

ALSは運動機能を侵す進行性の疾患であり、最終的には動けなくなり、呼吸筋と咽頭筋の麻痺により死亡する。別の方法で栄養をとらせる、呼吸器を用いる、在宅看護など、いくつかの方法により生命を延ばしうる。
米国のウエストヴァージニア州のような田舎町では、患者はこの病気について詳しい医師のところからは距離がある。町当局もまたALSの病気の知識、経験が少なく、必要な器具も直ぐには利用できないかもしれない。
この講演では、田舎に住むALS患者のケアについて、患者、介護者、ホームヘルス機関、ヘルスケアの提供者の観点に立って論じたい。特に、僻地環境に関する事柄について述べたい。
